Political Committee Registration - KREF 010 (.pdf 417kb)

This form should be completed if a candidate (other than a slate) chooses to establish a campaign committee.

Waiver from Filing Candidate Election Finance Statement - KREF 011 (.pdf 27kb)

This form is completed when the committee is taking responsibility for submitting the financial reports for the candidate.   

Financial Disclosure Report - KREF 025 (.pdf 170kb)

This form is used by Justices and Judges of the Court of Justice, Commonwealth's attorneys, all candidates for those offices and persons appointed to a complete and unexpired term of any of the offices mentioned above.  This form requires reporting of financial information as required by KRS 61.740.

Slate of Candidates: Appointment of Campaign Treasurer - KREF 001/G (.pdf 415kb)

This form is used by slates to appoint a campaign treasurer and to disclose the location of the campaign depository.

Gubernatorial Slate Campaign Committee Registration - KREF 010/G (.pdf 28kb)

This form is used only by Slates to officially file campaign committee information.

Notice of Gubernatorial Slate Replacement - KREF 034/G (.pdf 28kb)

This form is used only by Slates to name a replacement to the slate after a vacancy has occurred and the Secretary of State has certified the vacancy.

Waiver from Filing Candidate Election Finance Statement - KREF 011 (.pdf 27kb)

This form is completed when the committee is taking responsibility for submitting the financial reports for the slate.  Both the candidate for Governor and the candidate for Lieutenant Governor should complete a separate Waiver from Filing Form.
